Craft Physical Therapy does not contract with insurance companies. We choose to be an out-of-network provider because it allows us the freedom to provide better care without the restrictions and limitations of a traditional insurance-based model. This is considered "fee for service", which means that payment is made in-full at the time of service. You will receive a Super-Bill, which can be submitted to your insurance company for reimbursement depending on your individual plan. As a medical expense, you can also use your HSA or FSA account to pay for physical therapy services. We accept payment through cash, check, credit card, HSA/FSA card, and other digital payment platforms.

As insurance plans with high deductibles and HSAs or FSAs become more popular, seeking care from out-of-network providers puts you in control. And quite often, going out-of-network will save you money through transparent pricing and a model of higher quality one-on-one treatment. Don't let your care be dictated by non-clinicians sitting at a computer following a standard algorithm that has nothing to do with your individual situation. Take control of your health and make the choices that are best for you.

The great state of Utah recognizes the advanced skills and training required to be a Doctor of Physical Therapy. Through what is called Direct Access, we are able to evaluate and treat you without a physician referral. Direct Access promotes a cost-efficient and time-efficient model that allows you to take control of your health and recovery. By seeing a physical therapist first for common musculoskeletal conditions, you're able to receive care faster and without the extra cost and time of a physician visit. Physical therapists are trained in differential diagnosis to identify conditions that are and are not appropriate for physical therapy. Should you present with a condition out of the scope of practice for a physical therapist, you will be referred to the appropriate clinician for further evaluation.
